You may attain a certificate upon completion of the certification OR re-certification process if you wish. This is OPTIONAL. Please follow the information below to request your certificate from National University of Health Sciences.
COX® CERTIFICATION CERTIFICATE PROCESS
Dear Julie,
Our certificate request forms are now available on-line on our website, www.nuhs.edu (click on "continuing education"). To receive a program certificate, participants must:
1. complete the form by typing (not handwriting) in all the appropriate boxes
2. print the form
3. sign the form at the bottom
4. return the form to us by fax or mail
These forms are available now and will be the only forms accepted for programs beginning in 2007.
